For new patients, please check our lists of accepted Insurance, you may call our office if your insurance is not on the lists. Please bring with you any old shot record, Insurance card and driver's license.

To reduce your wait time, you may print the new patient registration forms and bring them to your visit.

Do you require forms filled? You may view this then email us to have the form ready for you for pick up. Please include your child's full name and date of birth on your request. reception1@childrenshcmd.com
