To save time let's make it simple: Schedule a physical to check how well you have maintained over the last year, catch up on the routine vaccines and refill medications all during your physical. Check your PORTAL acct to see if your child needs a physical. Your physical exams can be used also to complete a sport and daycare form. Its valid for 1year. We can do VIRTUAL VISIT in the comfort of your house for rashes, pink eye, Asthma Medications refills, ADHD (hyperactivity), follow up and medications, allergy medications refills and more! We can do Weight Management and Nutritional Counselling also virually, jus ask the front desk for a virtual visit or email through the portal. We would like to avoid the pressure for both you and us for not getting your child's forms in a timely manner, so please use the portal for easy downloads. Registered portal users can see their children's growth chart, print shot record, school and camp forms. There is a $10+ fee for all forms needing manual input with a 2-5days process time. We can use your physical if it's within a year to fill your forms, otherswise a new physical is needed. Keep Safe and keep Healthy! Dr. O
